Bootstrap to deliver Support Planning and Brokerage (December 2009)
Bootstrap has taken a major step towards involvement in the agenda of personalised social care by agreeing with Blackburn with Darwen Council to deliver a programme of support planning and brokerage for local people. Working alongside Age Concern Blackburn, Bootstrap is supporting Blackburn with Darwen Adult Social Services in its work to meet the target set nationally for the percentage of its social care users who have support plans. Bootstrap and Age Concern Blackburn now have a single dedicated team of trained support planners who take referrals directly from Adult Social Services. They work one to one with each of the referrals to help them put together meaningful support plans which can offer increased independence, choice and control in the way their social care is delivered.
